Terms & Conditions
- Clients are asked to complete the Consent and Medical Information Form either on Power Diary when booking or in paper format. If it is your first appointment with Derbyshire Physio it is beneficial to liaise with your therapist prior to your appointment if you have answered ‘yes’ to any of the questions, as this may mean we are unable to assess or treat you on the day.
- As prompted on the Consent and Medical Information Form if you answer ‘yes’ to any of the questions please contact Derbyshire Physio to discuss. You may be asked to provide a letter from your GP regarding fitness for Sports Massage if the medical complaint is deemed as a contraindication or precaution to massage. Please note failure to do this in advance may lead to Derbyshire Physio not being able to complete your massage, however, as the slot has then been utilised you will still be charged for this time.
- Each session will only last the duration of time that has been allocated. In order to provide the best service to all clients, we cannot commit to extending any particular session.
- Rates for Derbyshire Physio services are subject to change. Services prepaid for by the client, which are unused at the time of any rate change, will be honoured at the price already paid.
- In order to provide the best possible service to all clients, we ask clients to be ready to begin their session at the scheduled time. Time lost at the beginning of a session due to lateness cannot be made up at the end of the session as that could impact the next scheduled appointment.
- If you are having a ‘mobile’ appointment please take into consideration traffic, ease of parking etc. If any parking permits are needed or payment for parking, please inform Derbyshire Physio prior to initial visit
Medical Condition Changes
- By booking, you agreed to disclose the relevant information within the Medical Consent forms.
- It is your responsibility to advise prior to the cancellation period if any of the conditions listed within the consent forms change.
- Should you advise of the condition within the cancellation period or at arrival at the clinic, you will be liable for the relevant cancellation fee
Cancellations and Bookings:
- All cancellations must be made outside of 48 hours in order to avoid a cancellation fee
- Cancellations made inside 48 hours of the initial appointment will result in the client being charged the deposit fee
- Cancellations made inside 24 hours of the initial appointment will result in the client being charged for the full session. This is due to the potential inability of that session being filled on short notice
- Cancellation of a session within a ‘block booking’ will need to be rearranged within the same week to avoid rescheduling of all further booked sessions. If a client wishes to rebook outside of this timeframe this is at their discretion but please be aware that subsequent bookings are unlikely to be altered unless Derbyshire Physio is able to accommodate this.
Cancellations must be made by call or text to be deemed effective.
- If Derbyshire Physio needs to cancel a scheduled session the client will receive a credit for this session.
- All payments are to be paid in advance to the session taking place.
- Payments paid via bank transfer will not incur any charge.
- Results of the treatment will vary by individual and Derbyshire Physio cannot 100% guarantee specific results.
- Derbyshire Physio respects your privacy. All information collected is treated as STRICTLY CONFIDENTIAL and we will not share or redistribute your information with any third party except as necessary as required by law. Any information gathered from a client is simply for our records.
